Neglected Non-United Lateral Humeral Condyle Fracture in a Ten-Year-old Child

Lateral humeral condyle fracture (LHCF) is one of the most common fractures of the elbow in children. We describe the case of a 10-year-old boy who presented with a neglected, nonunion LHCF 60 months after the initial trauma. He was treated by open reduction and internal fixation after reducing the lateral condyle to its anatomical location and fixing the fracture using two screws; no addition of a bone graft and no anterior transposition of the ulnar nerve was needed. At the 14-month follow-up, there was complete union of the fracture. The patient achieved full elbow flexion and extension as well as full forearm supination and pronation, and the carrying angle was corrected from a preoperative 140° to 160° at the last follow-up. Proper diagnosis and management of LHCF at the first presentation are crucial; however, if the patient presents with a neglected and nonunion fracture, open surgical fixation leads to satisfactory radiological and functional outcomes.
