This might stem, at least partially, from an unequal access to healthcare, and results in NHB patients having a shorter life expectancy than their NHW counterparts. To mitigate these biases, which probably originate from systemic racism, we sought to compare prostate cancer-specific mortality (PCSM) in NHB vs. NHW men matched based on their calculated other-cause mortality (OCM) risk. METHODS: The Surveillance, Epidemiology, and End Results (SEER) database was queried to identify NHB men and NHW men who were diagnosed with PCa between 2004 to 2009. This time period was selected to obtain a sufficient long-term follow-up. A Cox regression model was used to calculate the 10-year OCM risk using all available covariates. This calculated risk was used to construct a 1:1 propensity score matched (PSM) cohort of NHB and NHW patients. Then, a competing risk multivariate model tested the impact of self-identified race (NHB vs. NHW) on PCSM, after accounting for all available covariates. RESULTS: There was a total of 93,957 patients included in our final cohort, with 19,286 (20.6%) being NHB men and 74,571 (79.4%) being NHW men. The median (IQR) follow-up was 11.2 years (9.8-12.7). In the unmatched cohort, 10-year PCSM and OCM were 5.3% vs. 3.4% and 13.7% vs. 8.4% in NHB vs. NHW patients (all p-value <0.0001). The standard mean difference was <0.1 for all variables included in the PSM model, which included a good match. In the matched cohort, 10-year OCM was 16.8% and 13.5% in NHB vs. NHW (p-value <0.0001), while the 10-year PCSM was 5.9% vs. 5.7% (p-value = 0.2). On multivariate analysis, in the matched PCSM cohort, NHW men had a hazard ratio of 0.99 (95% CI: 0.89-1.09) compared to NHB men with a p-value of 0.8. CONCLUSIONS: Our results showed similar PCSM in NHB and NHW patients, when matched based on their calculated 10-year OCM risk. Our report is the first to indicate at a population-based level that after alleviating the known systematic biases against NHB men in the "real world".
