However, MRI texture features of CC have not been fully investigated. Delta-radiomics is a new method of analyzing changes in texture features that are comprehensively evaluated by radiomics. The aim of this study is to evaluate the differences in mpMRI findings between CC and non-CC using radiomics and delt-radiomics, along with subjective assessment. METHODS: Of the prostate cancer patients who underwent radical prostatectomy (RP) between 2017 and 2021, we analyzed consecutive 103 who underwent mpMRI preoperatively. The presence of CC within the largest cancer lesion were examined on RP specimens. MRI findings of lesions≥5 mm in diameter with CC foci (CCL) were compared with those of significant cancer lesions (Gleason grade group (GG)≥2)≥5 mm without CC foci (non-CCL). Texture features of T2-weighted images (T2WI), diffusion-weighted images (DWI), and fat-suppressed T1-weighted images (T1WI) before and around 34 secs after contrast administration were extracted by radiomics analysis. Pre- and post-contrast changes of texture features in T1WI were evaluated using delta-radiomics. Radiomics analysis was performed using imaging software, LIFEx to extract 32 second-order texture features. RESULTS: Forty-three patients had CC. Of the 25 CCLs (median diameter: 15 mm, GG 2/3/4/5: 5/13/6/1), 14 CCLs were identified by mpMRI (PI-RADS 3/4/5: 1/3/10). The remaining 60 patients had non-CC. Of the 45 non-CCLs (median diameter: 16 mm, GG 2/3/4/5: 32/9/2/2), 27 non-CCLs were identified by mpMRI (PIRADS 3/4/5: 7/8/12); there was no significant difference in PI-RADS scores between CCL and non-CCL. Radiomics analysis showed a significant difference in only GLCM-Correlation on T2WI (p=0.0077), and no significant difference in any of the texture features on DWI. There was no significant difference between CCL and non-CCL in the pre- and post-contrast changes on T1WI in texture features. CONCLUSIONS: The only difference between CCL and non-CCL in the subjective evaluation and in the texture features in mpMRI was the GLCM-Correlation of T2WI. The mpMRI findings of CCL and non-CCL are considered to be almost identical.
