Introduction: COVID-19 affected organ transplantation in France, with a 26% drop in 2020. Donors with positive SARS-CoV-2 were previously excluded. Although organs from asymptomatic COVID-19 positive donors are now accepted, lung transplants are an exception. The risks following lung transplantation (LTx) with positive SARS-CoV-2 PCR are still being studied. In this setting we present two cases of initially negative recipients becoming SARS-coV2 positive in the immediate post-operative course of LTx.
