Syndrome De Détresse Respiratoire Aiguë Compliquant Une Maladie De Still De L'adulte

RESUME La maladie de Still de l’adulte (MSA) est une maladie auto-inflammatoire complexe qui se manifeste le plus souvent par l’association d’une fievre, d’arthralgies, d'odinophagie et d’une eruption cutanee. Une pleuresie survient dans 20 % des cas mais les atteintes parenchymateuses pulmonaires n’ont jamais fait l’objet d’une etude specifique. Nous rapportons le cas d'une patiente chez qui la survenue d'un syndrome de detresse respiratoire aigue a ete une circonstance de decouverte. L'evolution favorable sous corticotherapie en fait un diagnostic a ne pas rater. ABSTRACT Adult Still's disease (ADD) is a complex autoinflammatory disease, most commonly presenting with fever, arthralgia, odinophagia, and rash. Pleurisy occurs in 20% of the cases but the pulmonary parenchymatous lesions have never been the subject of a specific study. We report the case of a patient in whom the occurrence of an acute respiratory distress syndrom was a circumstance of discovery of ADD. Because the clinical course of this disease is usually favorable with corticotherapy, this diagnosis should not be missed.
