Though most patients undergoing transcatheter aortic valve replacement (TAVR) have some degree of aortic regurgitation (AR), outcomes are not clearly defined in those with concomitant severe stenosis and moderate or greater AR (AS+AR). Despite mixed pathology, mortality and complications were similar between cohorts, as were post-TAVR valve gradients and paravalvular leak rates. The authors postulated AS+AR may compromise left ventricular (LV) recovery and seemed to expect worse outcomes. However, on multivariable Cox regression, AS+AR was not associated with an increased hazard of death.
