According to Vuorela et al.’s recent report, the interposition of a spiral graft constructed from an autologous great saphenous vein (GSV) appears to be a safe and feasible option to reconstruct the portal vein/superior mesenteric vein (PV/SMV) after invasion by malignancy treated by pancreatectomy.1 However, the spiral graft might induce blood turbulence because of the asymmetric diameter, increasing the risk of a PV/SMV thrombosis.2 Overall, 171 pancreaticoduodenectomies (PDs) with venous resection/reconstruction were performed.
