Results of ischemic mitral regurgitation repair with the IMR ETlogix? asymmetric ring

Abstract
Introduction and objectives: The presence of ischemic mitral regurgitation leads to higher mortality. Among the different treatment options, the IMR ring was designed to correct the specific alterations of ischemic mitral regurgitation. We report our experience with IMR ring in ischemic mitral regurgitation. Methods: Retrospective study of all patients operated between 2006 and 2022 with ischemic mitral regurgitation correction by simple annuloplasty with IMR ring. Clinical records have been reviewed and telephone follow-up has been carried out. Results: One hundred and eighty-five patients were included. 81% had ventricular dysfunction and 62.2% three vessel disease. 17.9% had moderate mitral regurgitation and 82.1% severe mitral regurgitation. 51.1% of the patients underwent urgent surgery, 64.4% had a recent myocardial infarction and 43.7% had heart failure. 96.2% also underwent myocardial revascularization. Hospital mortality was 2.7%. Survival is 92.5% at one year, 72% at 5 years, 54% at 10 years, and 26% at 15 years after surgery. 69.8% did not present residual mitral insufficiency and 25.1% mild insufficiency. Freedom from mitral reoperation is 98% at 15 years after surgery. Of the living contacted patients (63%), 91.8% remain in functional class I-II. Conclusions: The IMR ring is a specific ring for IMi correction, which allows IMi to be repaired effectively with a simple technique, without increasing initial complications, and achieving better long-term survival.
