¿Influye El Examen «médico Interno Residente» En La Enseñanza De La Medicina En España?

Abstract
Resumen: Introducción y objetivos: el examen Médico Interno Residente (MIR) es decisivo para los estudiantes de Medicina. Nuestro objetivo ha sido analizar la posible influencia del examen MIR en la formación de nuestros estudiantes. Materiales y métodos: encuesta anónima en Google Forms que se distribuyó entre los 275 estudiantes del 5° y 6° curso de la Facultad de Medicina de la Universidad de Cantabria y los 347 residentes del Hospital Universitario Marqués de Valdecilla con 9 preguntas genéricas y 2 más dirigidas a los residentes. Las 5 primeras, así como las 2 de los residentes, analizaban la influencia de la preparación del examen MIR; y las 4 siguientes, posibles cambios en el examen. Resultados: contestaron 121 estudiantes (43%) y 133 residentes (38,3%). Algo más de un tercio de los estudiantes y un quinto de los residentes afirmaron que la preparación al MIR influyó bastante-mucho en la dedicación a sus estudios. El impacto en los estudiantes de 5° fue muy inferior al percibido por los estudiantes de 6°, donde fue especialmente relevante en la elección/dedicación a los trabajos de fin de grado. Hubo unanimidad en que el examen MIR debería mantenerse, pero casi la mitad de los alumnos y residentes creían que se debería acortar el periodo entre el final de la carrera y el examen, y aumentar el peso de las notas de la carrera en el examen. Conclusión: la preparación al examen MIR influye negativamente en la dedicación y formación de nuestros estudiantes de Medicina, particularmente en el 6° curso. Estos resultados indican que deberíamos abrir un debate serio para poner soluciones a este problema. Abstract: Introduction and objectives: The “MIR” (postgraduate medical education) exam is crucial for our students. Our aim was to analyze the possible influence of the MIR exam in the training/dedication of our medical students. Material and methods: Anonymous questionnaire in Google Forms, which was sent to 275 medical students in their 5th and 6th academic years in the Faculty of Medicine of the University of Cantabria and to 347 medical residents of the university hospital Marqués de Valdecilla with 9 generic questions and two more specific for residents. The first 5 questions, as well as the 2 exclusive for residents, analysed the possible influence of the exam preparation and the last 4 possible changes in the MIR exam. Results: A total of 121 students (43%) and 133 residents (38%) answered the questionnaire. Globally, more than one-third of the students and one-fifth of the residents noticed a relevant influence of the exam in their medical training and dedication. This impact was lower in the 5th year students versus those in the 6th year, in whom the influence was relevant in the election/dedication to their end of degree project. The vast majority supported the exam, but around half thought that the time to the exam should be shortened and that the weight of the degree marks in the exam should be increased. Conclusion: The MIR exam preparation influences negatively the training and dedication of our medical students, particularly in the final year. These results indicate that it is necessary to open a serious debate to try to solve this problem.
