A Retrospective Comparison Between Ultrasonographic Kidney Dimensions and Growth in Children with Multicystic Dysplastic Kidney

To assess the effect of height and functional kidney vertical length upon anthropometric, laboratory, and ultrasonographic measurements of patients with multicystic dysplastic kidney (MCDK). A total of 104 patients with MCDK were included in the study. Systolic and diastolic blood pressure measurements, laboratory values (serum creatinine, urea, cystatin C, urinary protein, and creatinine), gender, age at diagnosis, accompanying disorders, weight, height, and body mass index, and ultrasonographic parameters (vertical length of the kidneys, difference between both kidney lengths) of patients were recorded. Patients were divided into two groups according to the localization of the MCDK. Demographic, anthropometric, laboratory, and ultrasonographic parameters were compared between the two study groups. There were 40 females (38.5%) and 64 males (61.5%). The mean age was 67.32 ± 55.5 years. Twelve patients (11.5%) had additional disease. The weight and BMI z -scores of the patients with right MCDK were higher than those with left MCKD ( p < 0.05). Height z -scores, functional kidney lengths, serum creatinine, urea, cystatin C, urine protein/creatinine ratio, and systolic and diastolic blood pressure values were statistically similar in study groups ( p > 0.05). The height z -scores were positively correlated with systolic and diastolic blood pressure and kidney functional length ( p < 0.05). The functional kidney lengths were positively correlated with age, weight, and height z -scores in both patients with right and left MCDK ( p < 0.05). Pediatric patients with MCDK should be examined for the presence of additional urinary tract anomalies. Right or left localization of MCDK seems to be not affecting renal function. The increase in functional kidney size might not presume the progression to chronic renal failure in children with MCDK. Cystatin C, urinary protein excretion, and ambulatory blood pressure measurements might be used to monitor renal survival in patients with MCDK.
