Key Points The results of this analysis suggest that long CTO lesions (≥20 mm) are associated with lower technical success and higher in‐hospital MACE independent of other predictors of CTO outcomes Adds additional data to allow operators discuss the individualised risks and benefits of CTO‐PCI Future studies required prospective core lab analysis of lesion length and outcomes.
