•The prevalence of anxiety, depression, and insomnia symptoms in pregnant women during the third wave of COVID-19 in Colombia were higher compared to the general population in Colombia before pandemic.•Threatened Preterm Labor and Intrauterine Growth Restriction were associated with mental health outcomes in pregnant women.•It is crucial to identify maternal-fetal conditions in pregnant women to mitigate possible poor mental health outcomes.
