54 Vulvar hematoma in puerper served in a maternity in the Western Amazon: Case report

Introduction: The postpartum vulvar hematoma affects 300/1000 births, it is a potentially serious complication, since it evolves quickly, to a tense, compressible and painful mass, it is usually bulky and determines great blood loss in the puerperal woman, leading to postpartum hemorrhage. childbirth. More severe cases can invade the perineum and gluteal region, which may displace the vagina and rectum. Objectives: To report a case of a case of vulvar hematoma in a puerperal woman attended at a maternity hospital in the Western Amazon. Methodology: The present work uses references from the bibliographic research, to search for information and report this case of vulvar hematoma in a puerperal woman. Case Report: TOS, 22 years old, 39s3d, seeks care due to contractions, PA: 100X60mmHg, BCF13 bpm, AU: 35cm, Vaginal touch: 80% off neck, 4 cm dilated, cephalic, De lee -3. HT 32.1 HB 11.She was hospitalized and requested vaginal delivery, live NB, APGAR 9/10.Revised birth canal, presented grade II laceration, performed raffia. She was referred to the rooming-room, developed with syncope followed by fainting, suffering a fall from her own height. Physical examination: regular general condition, pale 2 + / 4, voluminous and painful vulvar hematoma. PA 80/40 mmHg. She was referred to the operating room for drainage of the vulvar hematoma. He has a good evolution in the postoperative period, he is discharged from the hospital with referral to a follow up clinic. Conclusion: Vulvar hematoma can have functional and aesthetic implications for the puerperal woman and should be avoided based on an adequate revision of the birth canal and suture in layers. In addition, frequent and attentive observation of the fourth delivery period is of fundamental importance for the rapid identification of the problem and intervention as soon as possible.
