Screening for risk and protective behaviors is recommended during adolescent well visits. Optimal approaches to screening have not been well described. Aims of this pilot study were: 1) to adapt an existing paper-based screening tool for use as a web-based tool (eTeenQ), 2) to describe the percentage of adolescents who completed the eTeenQ and agreed to have their responses stored in the electronic health record (EHR), and 3) to describe the prevalence of selected risk and protective behaviors reported through the eTeenQ.
