Moderate Obstructive Sleep Apnea and Cardiovascular Outcomes in Elder Patients: a Propensity Score Matched, Multicentric Study (CPAGE-MODE Study)

Aim: To determine the effect of continuous positive airway pressure (CPAP) on elders with moderate obstructive sleep apnea (OSA) regarding cardiovascular events (CVE) reduction. Methods: Observational and multicentric study of a historic cohort of elders (> 70 years of age) diagnosed of moderate OSA (apnea–hypopnea index 15.0 – 29.9). Two groups were formed: CPAP treatment and usual care. Primary endpoint was the occurrence of CVE after OSA diagnosis, and association with CPAP treatment was assessed by propensity score matching. Secondary endpoints were incidence of CVE separately (ischemic heart disease, atrial fibrillation, stroke) and time to first CVE. Results: A total of 614 patients were included. After matching, 236 elders (115 men, mean age 75.9 ± 4.7 years) were considered for endpoints purposes. Table 1 shows different characteristics of both study groups. Forty-one patients presented at least one CVE (17.4%): 20 in the usual care group (16.9%) and 21 in the CPAP group (17.8%), with an OR of 1.06 (CI95% 0.54 – 2.08, p=0.86) for CPAP treatment. No statistical differences were found in secondary endpoint analyses. Figure 1 shows Kaplan-Meier curves for time to first CVE. Conclusions: CPAP treatment did not reduce the probability of CVE in elders with moderate OSA.
