•Immunocompromised adults and patients living with chronic medical conditions are at high risk of pneumococcal disease.•In 2018, there were approximately 40 million adults in the main French national health insurance scheme and more than 4 million (10%) of them were at risk of pneumococcal disease.•About 18% of patients suffered from two or more conditions.•About 90% of them suffered from a chronic medical condition (diabetes, chronic respiratory disease, heart failure, etc.) and 14% were immunocompromised (immunosuppressive drugs, cancer, HIV, etc.).•Diabetes was the most frequent medical condition; patients with diabetes accounted for two thirds of all patients at risk of pneumococcal disease.
