EUS-directed transgastric endoscopic retrograde cholangiopancreatography (EDGE) is an established technique for performing ERCP in patients with Roux-en-Y gastric bypass. The 15mm lumen apposing metal stent (LAMS) has been utilized in the majority of published studies to date. The newly available 20mm LAMS theoretically renders the procedure easier by providing a wider lumen to enable same-session ERCP (or “single stage EDGE, SS-EDGE”). However, the larger stent might result in a higher rate of gastro-gastric (GG) or jejunogastric (JG) fistulas. No studies have compared EDGE outcomes via 15mm vs 20mm LAMS. Study aims: (1) rate of SS-EDGE procedures performed via 15mm vs. 20mm LAMS; (2) technical success (successful placement of a transmural LAMS followed by ERCP with cannulation of the intended duct and completion of the intended interventions); (3) rate of adverse events (AEs); and (4) rate of persistent GG/JG fistula.
