Giant Celiac Artery Aneurysm Treated with a Flow-Diverting Multilayer Stent: Early Rupture As a Fatal Complication

Institutional review board approval is not required at our institution for retrospective case reports. A 66-year-old woman with a giant celiac artery aneurysm who was treated with a flow modulator stent experienced an early rupture causing death. The aneurysm was detected incidentally during coronary computed tomography angiography. A large saccular celiac artery aneurysm (60 × 56 mm) with a wide neck measuring 11 mm was detected. The diameter of the parent artery proximal to the aneurysm was 7.1 mm, and the diameter distal to the aneurysm was 4.6 mm (Fig 1a, b). Endovascular repair was considered. The use of a covered stent was thought to be unsuitable because the stent diameter, which would have to be larger than the proximal landing zone, would have subjected the distal landing zone to an oversizing of > 65%. Stent-assisted coiling was not considered because of the giant size of the aneurysm and concern regarding being able to fill it to the point of occlusion. Following a review of the literature, a flow-diverting stent seemed feasible.
