Abstract
Limited data exist on the accuracy of echocardiographic (echo) colour doppler overlay mitral annular systolic excursion (CD-MAPSE; guided by B-mode echo) to estimate left ventricular (LV) systolic function (LVSF) compared to three-dimensional echo LV ejection fraction (3D-LVEF). CD-MAPSE could potentially provide a reliable and reproducible method for estimate LVSF. Appraise the diagnostic utility of CD-MAPSE to estimate LVSF (with 3D-LVEF as the reference) and determine interobserver reproducibility. Eighty-five patients having an indicated echo had assessment by 3D-LVEF per guidelines as well as CD-MAPSE. CD-MAPSE was calculated from independent septal and lateral mitral annular surface M-mode waveform calliper measures. Usability was evaluated by receiver operating characteristic curve (ROC) against the reference 3D-LVEF (ROC; ≤50% LVEF). Interobserver reproducibility was assessed using Bland-Altman analyses and mean absolute differences. Median age was 55±15yrs for patients (69% male) in sinus rhythm (68±12bpm). CD-MAPSE ROC found an AUC 0.77, 0.82 and 0.83 for septal, lateral and mean measures (cut-off criterions ≤11.0mm, ≤11.7mm, and ≤10.9mm). Sensitivity and specificity for septal, lateral and mean measures were 71% and 74%, 82% and 79%, 71% and 87% respectively. Septal and lateral ROC curves compared by DeLong method found non-significance (p=0.78). Interobserver reproducibility (n=30; 57±16yrs, 63% male) found mean bias of +0.8mm (3.5,+5.2), +0.8mm (-3.8,+5.5) and +0.8mm (-2.7,+4.4) for septal, lateral and mean measures, with mean absolute differences of 1.8mm (±1.5), 1.9mm (±1.6), and 1.5mm (±1.3) respectively. CD-MAPSE is reliable to estimate LVSF and demonstrates high diagnostic utility when compared to 3D-LVEF.
