•There is a knowledge gap on VNS battery end of service (EOS) in depression.•New onset depressive symptoms and side effects are associated with VNS battery EOS.•Clinical symptoms could be more sensitive than the software to predict EOS.•Prospective studies on clinical symptoms during EOS are therefore needed.
