Frequently patients with an intoxication due to the intake of methanol or ethylene glycol are admitted to Dutch hospitals. With relatively simple analytical techniques the toxicological laboratory can analyse a serum sample and answer the question what and how much was ingested. After methanol or ethylene glycol ingestion, ethanol is the antidote of choice. In case of a methanol ingestion haemodialysis is indicated when methanol concentrations are above 500 mg/l, or when metabolic acidosis or visual impairment are present. In case of ethylene glycol ingestion, haemodialysis is indicated when ethylene glycol concentrations are above 500 mg/l, when vital signs deteriorate, or when metabolic acidosis or crystalluria are present. The calculation of the optimal dose of the antidote ethanol is illustrated by a case report of a methanol and ethylene glycol intoxication.
