•The significant problems of poor health literacy are acknowledged worldwide.•Less is known about medication literacy and the desire to participate in decision making concerning medications, especially among groups of people with different levels of medication literacy.•This study adds understanding on the association of medication literacy and desire to participate in decisions concerning medication.•Thus, the results of the study are meaningful when involving and discussing with patients about the decisions concerning their medications.•Health and medication literacy and the desire to participate in pharmacotherapy decision making - comparison of two countries.
