Electrocardiograms (ECG) are becoming an important component of sports participation physicals for student athletes in middle schools, highschools, and colleges. Parents may choose to have these done at any medical center offering “sports ECGs” including but not limited to pediatric cardiology practices, primary care offices, and urgent care centers. Standardized criteria for the interpretation of ECGs in athletes have been published, however, in practice, the interpretation of sports ECGs is widely varied and dependent on who the reader is: pediatric cardiologist versus primary care physician; or in some cases practitioners may even rely entirely on the computer's interpretation. In order to have effective ECG screening programs; practitioners must be familiar with the subtle variations that can occur in adolescent ECGs and are commonly referred to as "normal variants".This article aims to provide case examples of the most commonly encountered variants in student athlete ECGs, to help guide clinicians to more accurately determine which require further cardiology evaluation.The key scientific concepts reviewed in this article are cardiovascular screening of student athletes and standardized ECG interpretation of student athletes.
