S of the XXIVlh Congress of ESAO Budapest, October 16-18, 1997 54 Our experiences with anatomical modular hip prostheses Tamas Meszaros M.D., Kalman T6th M.D. The long term stability of the total hip prostheses mainly depends on the perfect fitting and fixation of the prosthesis elements inside the preformed bone cavity. The condition of the primary stability is the anatomical congruity. Having made several anatomical examinations we planned and made hip prosthesis stems, which insure optimal fitting into various femur marrow cavities. We introduce the main characteristics of our anatomical-modular hip prosthesis system, and make clinical and radiological analysis and also some case reports of the results of 600 total hip prosthesis implantations. On the base of our excellent clinical and radiological results we recommend the use of this prosthesis system. 54 Acetabular revision with impact bone grafting, using acetabular reinforcement ring. 1. Semmelweis Medical University Budapest 4297 primary total hip replacement and 376 revision arthroplasty were performed from 1969 to December 1996 in the Orthopeadic Department of Semmelweis Medical University, Budapest, Hungary. Acetabular reinforcement ring and spongiosaplasty was the method in 105 cases following acetabular loosening with great bone loss. Authors give a description of this surgical technique and summary oftheir follow-up examination. According to the results the cemented acetabular fixation with acetabular reinforcement ring and spongiosaplasty seems to be a reliable method concerning the reestablished anatomical integrity and concerning the stability of the implant. 534 54 K. Department of Orthopaedic Surgery University Medical School of Debrecen and *Department of Orthopaedic Surgery of the Hospital of Kecskemet Authors applied succesfully cementIess press fit porous coated acetabular components in total replacement of 41 dysplastic hip joints without bone grafting, however in most cases according to Harris' concept using proximal placement of them without lateral displacement. 25 acetabular components were implanted proximally from the anatomical position, mean height of these components being 39,5 mm from the interteardrop line (the normal value 22 mm) however their horizontal location being equal to the normal values. Average follow-up was 33 months, range 18-60. Mean age of the patients was 42 year. The primary stability of the implants was satisfactory in all cases, no additional screw fixation was necessary and loosening or other severe complications were not observed during the follow-up period. 54 The Place of Surface Replacement Knee Athroplasty. Hungary The potential adventages of surface replacement knee arthroplasty (SRKA) include preservation of bone stock and cruciate ligaments and the nearly normal knee kinematics. In the past two decade's one and half thousand knee arthroplasty operations were performed in our department. The authors have made a retrospective comparison on the results of the SRKA performed in a ten year period between 1978-1987. From the operated 206 cases during that period it was possible to followup 109 cases (529 %).The mean follow-up period was 10.9 years (7-18 years). The mean age of the patients was 57.6 years (14-81 years) at the time of the operation. All types of the prosthesis consisted of modular metal femoral components and all polyethylen tibial components implanted with cement fixation. The results show that the SRKA has good results in cases of localised involvements, where the cruciate and collateral ligaments are intact Good long term results prove the success and effectiveness of the SRKA with correct indication and technique. While the SRKA is an excellent procedure with selected patients, at the same time it gives the possibility for the next step, should failure happen. In cases of extensive joint invlovement in systemic diseases (e.g. RA) and in the absence of ACL total condyler knee arthroplasty (TKA) is the most appropriate procedure. Than, V.Halmai, Department of Orthopaedic Surgery, Medical University Pees. One of the most important achievements of modern orthopaedics is the trappliance of the joirrt. This sort of operation has been carried out in 223 cases at the Department of Orthopaedics of the Medical University in Pees between 1993 and 1996 with the Johnson and Johnson PFC total knee prothesis. FJr ~he treatment of osteoarthritis of the patellofemoral joint during total knee replacement patella prosthesis has been applied in 23 cases, patellectomy was done in 17 cases. With most patients (183 cases) however, a solution less popular in international practice was chosen: spongialisation of the patella. Because of postoperative complications (infection, fracture) different kinds of revision surgery had to be carried out in 9 cases. In their presentation, besides describing the operative techniques, the authors reflect on the mid-term results observed at the control examination in 1997.
