The association between malpositioned metal-on-metal (MoM) implants and metallosis, high wear and increased concentrations of both cobalt and chromium particles and ions has been made from ion studies, retrieval analyses and hip simulator studies. This chapter reviews the range of clinical scenarios where implant wear and malposition were considered the primary causes for revision using five case examples. Risk factors in each of these failures are discussed.
