I am general internist and a health services researcher. My research aims to understand physician (and other prescribing clinician) practice variation to better deliver high-value care for aging adults with multiple chronic conditions. My ongoing work evaluates the role of physician integration within nursing homes in healthcare utilization by older adults. Using billing data from Medicare, I developed a rich database of clinical and healthcare utilization information for a cohort of patients that received post-acute care in skilled nursing facilities (short-stay patients). The database includes facility (hospital, nursing home, etc) and professional (physician and other providers) utilization. I also developed and tested claims-based measures of physician practice organization, including a measure of physician specialization in skilled nursing facility-based practice. My research agenda requires complex linkages of survey, Medicare claims, and other secondary datasets, and necessary methodological expertise in rigorous quantitative techniques to estimate causal effects using secondary data. My most recent work focused on mixed-methods research including secondary data analysis and primary data collection via interviews with patients, caregivers, and clinicians to better understand and improve the delivery of physician-level care for aging adults with multiple chronic conditions.
