P1-193: “delusion of abandonment (behave-ad-fw)” associated with poor self- awareness in ad: implications for delusional type-specific care and treatment

Delusion is a common symptom among patients with Alzheimer's disease. We previously reported that delusions of “theft”, “abandonment ” and “not my home” have their specific neurological backgrounds. The present study focused on these three types of delusions and clarified the content-of-delusion-specific associationswith particular cognitive domains. 109 consecutive outpatients, who were diagnosed as probable AD according to the NINCDS-ADRDA, no cerebral infarction asshown by MRI, and MMSE 9+ scores, were enrolledin the study. They underwent MMSE, Cognitive Abilities Screening Instrument (CASI) and Frequency-Weighted Behavioral Pathology in AD Rating Scale (BEHAVE-AD-FW) to assess cognitive function, delusion, respectively. The patients and their caregivers administered the Everyday Memory Checklist (EMC) to evaluate self- awareness of the disease, which quantifies the discrepancy of the EMC scores between patients and their caregivers. Spearman's correlations among each type of delusion were calculated. Multivariate logistic regression was performed to investigate associations between each delusion and general cognitive function, recent memory, orientation and judgment. 44 patients had delusion. “Delusion of abandonment” was significantly correlatedwith unawareness of the disease. “Not my home” was associatedwith recent memory, orientation, judgment and MMSE total score. A significant c orrelation was noted between the abandonment andthe theft. Our study suggests a heterogeneity of delusions, showing the content- specific associationswith particular cognitive domains. This suggeststhat content- specific treatment and care of delusion is needed.
