Endometriosis is a disease that almost always involves women in fertile age and that has a detrimental effect on the quality of life of the patients. Pelvic pain represents with infertility the main symptom of endometriosis. The aim of endometriosis treatment is to remove ectopic endometrium implants. It can be achieved only by surgery. Endometriosis medical therapy only improves symptoms but does not resolve the disease and when it is stopped endometriosis usually recurs. All the medical treatments suitable for pelvic pain associated to endometriosis have the same efficacy, nevertheless the most important part of them pre- sents significative side effects that prevent their long-term use. In patients with pelvic pain due to endometriosis that need long-term medical therapy, estroprogestins, often in association with nonsteroidal anti-inflam- matory drugs, at the moment are considered the first-line treatment for efficacy, risks and side effects incidence.
