We read with interest the paper by Tilki et al. [ [1] Tilki D. Chen M.H. Wu J. et al. Prostate-specific antigen level at the time of salvage therapy after radical prostatectomy for prostate cancer and the risk of death. J Clin Oncol. 2023; 41: 2428-2435 Crossref Scopus (0) Google Scholar ] on the association of prostate-specific antigen (PSA) level at the time of salvage therapy after radical prostatectomy (RP) with risk of death. Among patients with at most one high-risk factor, initiation of salvage radiation therapy at a PSA level of >0.25 ng/ml was associated with higher all-cause mortality risk. While we congratulate the authors for exploring an important research question, we believe that several factors should be considered before drawing such a conclusion.
