Reach and effectiveness of a centralized navigation program for patients with positive fecal immunochemical tests requiring follow-up colonoscopy

•Patient navigation led to high colonoscopy completion after abnormal stool tests.•Community health center patients were largely willing to participate in navigation.•All uninsured patients accepted navigation and completed follow-up colonoscopy.•The navigator successfully addressed multiple types of barriers to colonoscopy.
