•Head and neck squamous cell carcinoma have poor prognosis due to metastasis.•microRNA-21-5p, hypoxia, and cancer associated fibroblasts may lead to metastasis.•Hypoxia promotes microRNA-21-5p levels in exosomes that transform fibroblasts.•microRNA-21-5p increases invasion and metastasis both in vivo and in vitro.•microRNA-21-5p targets YOD1 in cancer associated fibroblasts to promote metastasis.
