Multidirectional instability (MDI) of the shoulder, typically an atraumatic condition, refers to the ability to dislocate or subluxate the glenohumeral joint anteriorly, posteriorly, and inferiorly. Patients with atraumatic MDI generally have pathologic laxity contributing to their instability. Treatment is initially conservative. For those that fail conservative management, the open inferior capsular shift remains a powerful tool for correction of MDI.
