•Intrathoracic surgery for advanced ovarian cancer is both safe and feasible.•Complete gross resection of ovarian cancer can be obtained in patients with intrathoracic disease.•Primary cytoreductive surgery with intrathoracic procedures can offer excellent survival outcomes in select patients.
