•Doxycycline has been reported to have side effects and treatment failures when used as a first-line agent in scrub typhus.•Azithromycin has been reported to be a good alternative to doxycycline in the treatment of scrub typhus.•Therapeutic effects of intravenous azithromycin are comparable to those of oral doxycycline in patients with scrub typhus.•Intravenous azithromycin can be recommended as a good alternative agent when oral doxycycline is contraindicated.
