•277,459 (128 ICUs) Brazilian/Uruguayan and 164,399 (84 ICUs) Dutch admissions were analysed.•The Rapoport-Teres efficiency matrix is easy to interpret but limit further statistical analysis.•Averaging SMR&SRU, ASER, is a potential alternative single efficiency metric when correlation is high between them.•As a continuous metric, ASER may be more suitable in studies to identify targets to improve ICU processes and efficiency.
