septum, with areas of increased attenuation interposed with areas of decreased attenuation, creating a mosaic perfusion pattern (Fig 1C).Two CTPAs performed after 6 months (due to persistent dyspnoea) and 9 months (worsening dyspnoea) revealed residual persistent bilateral PE.Two sets of APS antibodies performed 9 months apart were negative.A diagnosis of CTEPH secondary to primary "seronegative" APLS was made.Conclusions: "Unprovoked" PE in young individuals needs further attention.APS, including seronegative APS, should be considered.The persistence of PE symptoms warrants further investigations as chronic thromboembolic pulmonary hypertension (CTEPH) may be the cause.
