•In a large German cohort of patients with rheumatic diseases we find a general good outcome of pregnancies under special surveillance.•Prematurity occurs more frequently in patients with connective tissue diseases. There was no significant increase in fetal complications or neonatal stress parameters, which emphasizes the importance of the special antenatal care in this group.•In our registry, patients with inflammatory joint disease flare more frequently than patients with connective tissue diseases and therefore might also be offered a closer monitoring during pregnancy.
