•We evaluated the second primary cancer (SPC) incidence and risk factors after oral cavity cancer (OCC).•OCC survivors face an increased risk of SPC.•SPC occurs in a site specific and time specific manner.•A younger age at diagnosis and radiation therapy were risk factors for SPC.
