•EAMG animal model proves that LRP4 auto-Ab is the third cause of MG.•Both the inhibition of agrin signal and activation of complement system are involved for NMJ disassembly in LRP4-EAMG.•The mechanism study suggests a similar clinical treatment between LRP4+ and AChR+ MG patients, but not MuSK+ ones.
