•Pineal cysts were identified in 55.6% of a small sample of adults who stutter.•Pineal cysts were identified in 5.3% of people with no known stuttering.•This is the first gross structural anomaly associated with stuttering.•Shared pathogeneses of pineal cysts and stuttering are proposed.•Researchers are encouraged to reexamine scans of PWS to look for pineal cysts.
