•Fifty percent patients with tuberculous meningitis (TBM) had cerebral infarctions on MRI.•A total of 42.3% of patients with infarction had intracranial MRA abnormality.•None had cerebral venous sinus thrombosis (CVST) on MRV.•Infarction in TBM was arterial in origin and not due to CVST.
